Born in London on 20 October 1866, second child and eldest son of Paul Augustus Hepburn, member of a firm of tanners, and Mary his wife, daughter of Henry John Langton. Mrs Hepburn's youngest brother was John Langton, surgeon to St Bartholomew's Hospital. P A Hepburn was the eldest son of his parents and, as he died young, his son became head of his branch of the "clan". Malcolm Hepburn was educated at Uppingham and at St Bartholomew's, where he won the Foster prize in 1888. He specialized in ophthalmology and joined the staff of the Royal London Ophthalmic Hospital, Moorfields, where he became dean of the Medical School and ultimately consulting surgeon. He served for a time as assistant surgeon at the Central London Ophthalmic Hospital, and was also consulting ophthalmic surgeon to the Royal Free Hospital. He was lecturer in ophthalmology at the London (RFH) School of Medicine for Women, and examined in ophthalmology for the Conjoint Board in Part 2 of the Diploma, 1922-27. He practised at 111 Harley Street. Hepburn served on the council of the Ophthalmological Society of the United Kingdom, became president of the section of ophthalmology at the Royal Society of Medicine, and delivered the Doyne memorial lecture at the Oxford Ophthalmological Congress 1935. He was a Fellow of the Royal Geographical Society and a member of the Alpine Club. Hepburn married on 30 September 1896 Ruby Worthington, who survived him with a son and three daughters.
